Two Ill. Producers’ Licenses Revoked
Illinois Director of Insurance J. Anthony Clark has denied the insurance producer license application of Philip A. Fair of Chicago effective June 16. Fair was previously licensed to sell life, accident and health, and fire and casualty insurance by the department from 1992 to 1999.
Clark also revoked the license of Thomas V. Conwell of Lake Bluff, effective June 13. Conwell had been licensed since March 8, 1993 and was licensed to sell life, accident and health and variable insurance.
Fair’s license application was denied, the Insurance Department said in a statement, as a result of an investigation which revealed he provided incorrect, misleading and materially untrue information on his application for a license, previously sent a check with insufficient funds to the department, failed to respond to the department on a consumer complaint, failed to provide books and records, and failed to a hold premiums in a fiduciary capacity.
Conwell’s license was revoked by director’s order from Hearing #3991 regarding the department’s Order of Revocation dated April 30, 2002. The stated Order of Revocation was issued, the Insurance Department said, as a result of an investigation which revealed that the licensee submitted documents containing non-genuine signatures of insureds to an insurer, rebated premiums, failed to repay a loan he obtained from an insured, and that his registration as a securities salesperson had been revoked by the Secretary of State Securities Division.
Conwell appealed the case for review under the Illinois Administrative Review Law; however, that case has been dismissed. Fair still has the right to appeal the denial of his license.
