Miami (Okla.) Counselor Charged with Medicaid Fraud
Donna L. Wallace was charged this week in Ottawa County District Court with one felony count of Medicaid fraud after an investigation by Oklahoma Attorney General Drew Edmondson’s Medicaid Fraud Control Unit.
Wallace, 60, is accused of forging signatures on at least seven treatment plans and treatment plan reviews that were submitted to the Oklahoma Foundation for Medical Quality and subsequently approved for Medicaid eligibility.
Wallace is a former counselor for ROCMND Area Youth Services in Miami. ROCMND is a treatment facility servicing Rogers, Ottawa, Craig, Mayes, Nowata and Delaware Counties.
The state alleges Medicaid was billed approximately $9,000 for false claims submitted by Wallace.
